Insurance information

We accept traditional Medicare and many commercial insurances. If you have a Medicare Advantage program (even if it is a PPO), please be aware we cannot guarantee it will cover our services. If you have an insurance that does not cover our services, you will be responsible for payment of services.

As of 3/1/2022, we no longer accept Humana insurance. As of 7/1/2023, we no longer accept Medicaid. This includes if these insurances are your secondary. If you have these insurances, and would like to be seen in our office, you must pay in full at the time of the appointment, and we will provide a statement that you can send to your insurance company for possible reimbursement. Please note: we do not guarantee you will receive reimbursement for any services rendered.
